School Caregiver of the Year Award 2025

Diabetes Youth Services is excited to honor and celebrate our school nurses and professionals who go above and beyond to ensure that students with type 1 diabetes can thrive in the classroom. The “School Caregiver of the Year Award” is a recognition of the exceptional dedication, compassion, and support demonstrated by school nurses and other professionals vital for students living with type 1 diabetes. The recipients of this award are educators, nurses, and school staff who create safe, inclusive, and supportive environments for students with Type 1 diabetes. They work tirelessly to monitor blood sugar levels, administer insulin, educate students and staff about diabetes care, and ensure that the student’s medical needs are met while fostering independence and confidence in managing their condition. These caregivers play a crucial role in making sure that students with Type 1 diabetes can participate fully in school activities, excel academically, and remain engaged with their peers, all while prioritizing their health and well-being. The School Caregiver of the Year Award celebrates these individuals’ remarkable commitment to not only providing medical care but also nurturing a sense of belonging and community within the school.

 

THE WINNER WILL RECEIVE: A personalized engraved award, a feature on DYS’s website, social media, and newsletter, and a permanent jewelry bracelet, sterling silver or 14kt gold (up to $65 in value) from Spoil Me Royal in Maumee. 

 

NOMINATION RULES: 

-One nomination per family. The number of nominations is not accounted for when determining who the winner will be.

-The nominated professional must work at a school in our 37-county service area. Visit our website to view all qualifying counties in NW Ohio and SE Michigan HERE.

-Nominated staff member must be currently employed at the school your student attends. Not a retired employee or an employee who no longer works for the school.

-Nominations will be accepted from April 7, 2025, to May 9th, 2025, at midnight. We will not accept nominations after this deadline.

-The winner will be announced no later than May 19th, 2025.

-We ask that you only recognize and reward dedicated school staff, not talk badly about other staff members or bad experiences. This is for both the form below and the public posts on DYS social media.
