Diabetes Youth Services celebrates our school nurses and professionals who supports students in the classroom. We invite our awesome families to nominate those who go above and beyond for local students with diabetes.

WINNER WILL RECIEVE: Personalized Award with Engraved Name | Feature on DYS Website and Newsletter | Beauty Experience Package provided by Serenity Health and Wellness Center in Maumee, OH

RULES:

-One nomination per family. The number of nominations does not necessarily determine who the winner will be! We will take into account any school caregivers who handle multiple diabetic students.

-School must be located within our 38-county service area. Visit our website to view all qualifying counties in NW Ohio and SE Michigan.

-Staff member must be currently employed at the school your student goes to- not retired, etc. 

-Nominations will be accepted from April 15, 2024 to May 10, 2024 at midnight. We will not accept nominations after this deadline, even if the form remains open.

-Winner will be announced no later than May 10, 2024. We will announce via social media, DYS website, and directly call the winner and their nominator(s).

-No put downs. This is a time to recognize and reward dedicated school staff, not talk badly about other staff members or bad experiences. This is for both the form below as well as public posts on DYS social media.
